2025 Club Cancellation & Make Up Request Form
Please use this form to submit only cancellation and make up requests. Any additional questions please email us directly. You will receive a copy of this request to help you keep track of your schedule changes.
Cancellation and Make Up: Please let us know both date of Cancellation and Make up.
Cancellation Only: If you do not have a Make Up date yet.
Make Up (or Previous Cancellation): Please indicate Make Up Date and which Cancellation you are using.
Our make up policy is important to help us maintain the quality of our training. It is available during Winter/Spring/Fall only.
Make up days are permitted and based on schedule availability, cancellations must be submitted to our club cancellations/make up form a minimum of 3 hrs prior to the session 24hrs is preferred. In return you will have a make up day and 30 days from your cancellation to contact us to use your day, otherwise the make up will expire (we do not allow walk ons all visits must be scheduled in advance).
These make ups session requests must be submitted via club Cancellation/Make up form at minimum 24hrs prior to the session. Canceled make ups will revert back to the original cancellation date. There are no exceptions, we value every athlete and strive to maintain our training standards by keeping a strict coach to athlete ratio. We appreciate everyone’s help in maintaining a strong club.
When we look at the schedule we decide how many coaches we need to send for each day. If you do not show and do not notify us, Jake still has to pay that coach. So you can see why with how large the club is right now why this policy is very important.
